Instructions for Completing
Schedule and Class Information Sheets
- Once you have established your
schedule, complete the following Schedule and Class
Information Sheet.
- Column 1 - The sheet is divided
into a 6 segment day.
- Column 2 – Indicate the type of
instructional setting
- (Co-taught or Resource or
Self-Contained). If it is a Co-taught class, please
indicate the name of your partner teacher.
- Column 3 – Indicate the Subject
taught for that segment.
- Column 4 – List the names of the
students with disabilities.
- Make a copy for your file.
- You will be asked to provide this
information at the beginning of the school year and prior to
each FTE count date.
Send the first completed Scheduling Sheet to Dr. Sharon Dodds
no later than August 15, 2008.
